Forms may be completed online, saved to computer, and e-mailed as attachments to Janet Allen Crocker (allen_jh@mercer.edu) in Marketing Communications, or printed and completed by hand. Hand-completed forms may be delivered to Marketing Communications by fax (301-4124), campus mail, or in person.
