CANDIDATE INFORMATION FORM
All Tift students must complete this form in their LiveText accounts. EACH SEMESTER, WE ASK THAT YOU CHECK YOUR FORM TO BE SURE THE INFORMATION IS STILL CORRECT. Please be careful to identify your program correctly and NOT to provide answers for any questions that are not applicable to you. To complete or check your information in this form, do the following:
- Login to your account and click the Forms tab. You should see four or more forms listed there. Ignore all but the one entitled “Candidate Information Form.”
- If you have already filled it out or it has been filled out for you, the title of the form will be a link – click the title to see the information that is currently in the form. If it is correct, do nothing.
- If it is incorrect, click the Go Back button at the bottom of the form and return to the list of forms, then click the Take Again link; complete the form with the correct information and Submit.
- If the title is not a link, that means your form is empty; click Take Form, then complete and submit the form. After you have completed this form correctly, you will not need to do it again unless you change programs.
- Guidelines for completing this form:
- No form should have information for both Initial AND Advanced programs. Complete ONLY the Initial Certification Program field OR the Advanced Program field, not both – leave the other one blank or N/A. (Skip these fields entirely if you are only in an Add-On or Endorsement program – see b.)
- Candidates in one of the endorsement or add-on programs should also complete the Add-On/Endorsement . If you are ONLY in the endorsement or add-on, the Inital or Advanced Program fields. If you are doing both at the same time (i.e., an EDL add-on or ESOL or Reading Endorsement, plus the EDS Teacher Leadership degree or The Holistic Child degree program plus the ESOL Endorsement), complete both Initial or Advanced AND Add-On/Endorsement.
- If you change from one program to another, please make the changes in this form during the first semester you go to your new program. Candidates who complete one program (e.g. an initial certification program) and then enroll in another program (e.g. an advanced graduate program) should ensure that the current version of their completed form reflects the new program and that all of the old program information has been removed.
