Immunization and Health Documents
Georgia Baptist college of Nursing of Mercer University requires nursing students to submit medical information and updated immunization records. Complying with these requirements safeguards the health and safety of students and clients in the classroom and clinical setting. Students not meeting the specified requirements will be excluded from clinical and/or class. Students transferring from the Mercer University Macon campus to the Atlanta campus are expected to arrange for transfer of records from Campus Health in Macon to meet the additional requirements for nursing students.
The Campus Health Office is aware that old medical records are sometimes hard to obtain. Please write your dates on the Immunization Form and attach documentation as needed for older immunizations. Signatures are required of either the individual who has administered the immunization or photocopy of the record that the information is transcribed from. Questions should be directed to the Mercer University Atlanta Campus Health Office at 678-547-6130.
Immunization and health information forms should be forwarded to Campus Health as noted on the Student Record Checklist.
