IACUC Application Submittal Instructions

After the form is downloaded to your personal computer, use your word processor to complete the application. After completing the application, determine how you want to "sign" the application. If you use an electronic signature, you can return the form as an E-Mail attachment or on a CD. If you print the document and sign the last page you can return the form by FAX or mail. Return the completed application to the address below.

For Veterinary signature, please send a copy of the application by one of the following methods:
