IACUC Application: This form must be used to obtain initial approval from the IACUC to use animals in research, teaching, training, and for on-going investigations extending beyond three years.
IACUC yr2_3 Update No Changes : This form is to be used to obtain re-approval from the IACUC for investigations using USDA protected species which extend into the second or third year where no significant changes* have been made to the original approved study protocol (ASP).
IACUC Modification : This form is to be used to obtain approval for any significant changes to an approved study protocol.
Please be advised that any investigator, with an animal protocol, can have their protocol previewed by an Institutional Animal Care and Use Committee (IACUC) member or a Veterinarian.
