IACUC APP for Use Animals 2004 .doc- This form must be used to obtain initial approval from the IACUC to use animals in research, teaching, and training, and for approval of on-going investigations extending beyond three years.

IACUC yr2_3 Update No Changes ASP 2004.doc - This form is to be used to obtain re-approval from the IACUC for investigations using USDA protected species which extend into the second or third year where no significant changes* have been made to the original approved study protocol (ASP).

IACUC Modify ASP 2004.doc - This form is to be used to obtain approval for any significant changes to an ASP at any time during the approved duration period of a study.
Please be advised that any investigator, with an animal protocol, can have their protocol previewed by an Institutional Animal Care and Use Committee (IACUC) member.
