After the form is downloaded to your personal computer, use your word processor to complete the protocol. After completing the protocol, determine how you want to "sign" the protocol. If you use an electronic signature, you can return the form as an E-Mail attachment. If you print the document and sign the last page you can return the form by E-mail or FAX.                                                                  

The application must be reviewed and signed by a Veterinarian before IACUC review. For Veterinarian signature, please send a copy of the application to:
